Low-level laser for treatment of tinnitus: a self-controlled clinical trial
BACKGROUND: Despite the high prevalence and morbidity, tinnitus still remains an obscure symptom. We assessed the
efficacy of low-level laser for treatment of tinnitus.
METHODS: It was a self controlled clinical trial study on 61 outpatients with subjective tinnitus. The patients were irradiated
with a 650-nm, 5-mW soft laser for twenty days and twenty minutes per day. The sensation of tinnitus was
measured on a Visual Analog Scale (VAS) before and two weeks after treatment and they were compared by means of
Wilcoxon signed rank-test.
RESULTS: Thirty-eight (62.3%) patients were men and twenty-three (37.7%) were women. Fourteen patients (31.8%)
worked in noisy environment. The VAS mean difference before and after the treatment was statistically significant
(p < 0.0001). The best treatment effect was in the youngest group and there were significant differences between this group
and the middle age and older groups (p = 0.018 and 0.001, respectively). The mean VAS score reduction was not statistically
significant between male and female patients (p = 0.23). Also, the treatment outcome according to the noise level in
patient’s workplaces was not significantly different in women (p = 0.693), but it was significant in men (p = 0.029).
CONCLUSIONS: Transmeatal low-level laser irradiation is effective for the treatment of tinnitus and some variables like
age and job can affect the treatment outcom
THE USAGE OF MEDICAL INFORMATICS IN CRITICAL CARE MEDICINE
Introduction: The intensive care unit can be defined as a complex system that composed of clinical informations, tasks and knowledge. It is also one of the most stressful and most vital parts of healthcare centers which involve a huge amount of information and clinical data daily. These informations should be analyzed and managed in the best way as soon as possible to restore patients to normal by health professionals. Accurate and timely diagnosis, the best treatment and to avoid any possible error in this section can be equivalent to achieve the best possible result and reducing the length of hospitalization and mortality. As a result, due to the complex nature of the critical care and the mass of clinical data; using a combination of medical knowledge with the latest technologies and use of medical informatics capabilities can be the best way to reduce the workload of the sector and improve the quality of patient care.
Methods: A systematic search was conducted on the PubMed/MEDLINE, web of science, BMJ, ScienceDirect, and Scopus database for finding studies that have related to critical care and usage of informatics or medical informatics. The collected data and results are summarized by researchers and the results analyzed based on similarities and differences.
Results: With increased development of medical informatics and electronic systems has led to substantial progress in the field of critical care since 1980 till now around the world. Several articles, research projects has published. From 1981 to 2016 almost 600 scientific papers were found which included reports, posters and thesis written in context of critical care medical informatics. More than 65 percent of these studies is about designing CDSS in this field. Today, many of these systems are used in medical centers around the world and lead to improving the quality of patient care and reduce medical errors in intensive care units. Among these 600 papers, 230 articles related to our issue in fields of the design of electronic records, clinical information systems, decision support systems, data mining, telemedicine, smart analysis of clinical information and knowledge extraction techniques were selected as the main source of this study.
Conclusion: According to results of our study, it seems that critical care is potentially a valuable resource for medical informatics researches. The applied of medical informatics in the different fields of the diagnosis, interpretation, and treatment in different countries have improved the quality of care for patients in critical care field. We can mention some common fields which used such as infection control and early detection in intensive care units, clinical information systems, and CPOE and decision support systems such as APACHE system for grading the severity of illness of patients who hospitalized and even telemedicine. Since this area of research has not been a field of interest in Iran yet, it seems that this study with the aim of review the application of medical informatics in different countries could lead to practical researches in this field

The association of thyroid disease and palmar keratoderma rarely reported. Hearing loss etiology in patients referring to Isfahan cochlear implantation center
Introduction: Hearing loss is the most common congenital disorders occurring among newborn. Identifying the factors affecting it would reduce the incidence of this disorder. Therefore, the aim of this study was to examine the etiology of congenital hearing loss in patients referring to Isfahan Cochlear implantation center. Materials and Method: This study was performed on 689 patients with cochlear implantation. Demographic data and relative frequency of different causes of congenital hearing loss (acquired and genetic) were determined and recorded. T-test, Chi-Square and Mann-Whitney tests were used to compare the variables studied. Results: Our findings showed that 50 patients (7.7%) had history of drug use, 9 (1.3%) had history of taking ototoxic drugs, 99 (14.3%) had history of kernicterus, 157 patients (22.8%) had a history of hyperbilirubinemia, 15 (2.1%) history of meningitis, 57 (8.3%) had a history of seizure, one (0.1%) had a birth weight less than 1500 grams, 4 cases (0.06%) had history of hypoxia, 18 cases (2.6%) had history of trauma, one (0.1%) had CMV history, and one (0.1%) hearing loss due to syndromicity. On the other hand, it was determined that the probable causes of hearing loss were not related to the severity of hearing loss and age of the patients (p <0.05) Conclusion: Although the prevalence of congenital hearing loss is low, it is important to identify and screen for postnatal congenital hearing loss, especially in people with risk factors known in this study, including patients with kernicterus.
Lateral rectus muscle recession and the vertical palpebral fissure height: to do or not to do inter-muscular septum dissection?
In this study, we aimed to evaluate changes of vertical palpebral fissure height (VPFH) after unilateral lateral rectus muscle recession. Twenty-five and twenty-six patients who were candidates for lateral rectus muscle recession were assigned into "with" and "without" intermuscular septum dissection study arms. The VPFH was measured at one-day before surgery and in two weeks and three months, postoperatively. Three months after surgery, significant increase of VPFH was observed in both groups (Paired t-test; P=0.005). Also, less widening of VPFH was observed in "with intermuscular septum dissection" group (Change in VPFH in "with intermuscular septum dissection" vs "without intermuscular septum dissection" groups: 0.48 mm vs 1.34 mm; ANCOVA test; P<0.001). However, such results were not observed two weeks post-operatively (Change in VPFH in "with intermuscular septum dissection" vs "without intermuscular septum dissection" groups: -0.28 mm vs 0.28 mm; ANCOVA test; P=0.302). Intermuscular septum dissection is recommended in lateral rectus muscle recession to partially prevent the undesirable increment of VPFH
Baseline subfoveal choroidal thickness as an indicator for bevacizumab outcomes in central retinal vein occlusion
Background: Retinal vein occlusions are one of the most common form of retinal vascular disorders and could lead to vision loss due to macular edema, macular ischemia and sequelae from neovascularization. Anti-venous endothelial growth factor (anti-VEGF) treatment is the choice strategy of treatment for patients with macular edema secondary to central retinal vein occlusion (CRVO). There is an evidence of body with the controversies regarding increment of choroidal thickness in CRVO condition. The current study was designed to determine whether baseline subfoveal choroidal thickness may be an indicator for visual and anatomical outcome of bevacizumab in patients with CRVO macular edema. Methods: This study was a prospective clinical cohort study that enrolled in 23 new cases of treatment-naïve central retinal vein occlusion (CRVO) from February to July 2017 who were visited in Feiz Eye Hospital, Isfahan, Iran. Patients received a single injection of bevacizumab and were followed for 30 days. Ratio of subfoveal choroidal thickness (SFCT) was measured using enhanced depth imaging spectral-domain optical coherence tomography (EDI-OCT). Ratio of SFCT of the CRVO eye to the fellow healthy eye (SFCT1/F) was taken as independent variable. Changes of best-corrected visual acuity (BCVA) in LogMAR (ΔBCVA, functional response) and secondary to baseline central macular thickness ratio (CMT2/1, anatomical response) in the CRVO eyes were taken for comparative and correlative analytics. Results: A total of 46 eyes from 23 patients with the mean age of 64.60±10.19 years were included in this study. Baseline SFCT was higher in CRVO eyes (251.91±46.09 μm) in comparison to the fellow eye (206.95±26.62, P<0.0001). Also central macular thickness in CRVO eyes were significantly higher in CRVO eyes in comparison with fellow eye (531.04±38.22 vs 303.30±33.59, respectively, P<0.05). SFCT1/F, correlated moderately with anatomical (CMT2/1) and strongly with functional response (ΔBCVA). Conclusion: Bilateral evaluation of SFCT by EDI-OCT in all newly diagnosed CRVO cases is recommended to determine if there is a relative increase in choroidal thickness. Therapeutic effect of intratympanic injection of dexamethasone plus hyaluronic acid on patients with meniere's disease
Introduction: Intratympanic therapy, as a widely used treatment for inner ear diseases, is regarded as a therapeutic method in controlling the vertigo of the patients with Meniere’s disease. This study was designed to evaluate the effect of the Intratympanic dexamethasone-hyaluronic acid combination on patients suffering from Meniere’s disease. Materials and Methods: This study was a clinical trial on patients with Meniere’s disease during 2016-2017. Patients received two Intratympanic injections of dexamethasone plus hyaluronic acid as a mixture within a month. Before and 2 weeks after the intervention, pure tone average (PTA) at 0.5, 1, 2, and 4 KHz frequencies, speech discrimination score (SDS), dizziness handicap inventory (DHI), and tinnitus handicap inventory (THI) scores were evaluated for each patient. The obtained scores were statistically analyzed. Results: This study was conducted on a total number of 25 patients with Meniere's disease. The mean age of participants in this study was 44.71±4.92 years. Gender distribution among participants revealed that 36 of patients were male. The mean values of PTA, SDS, and THI were not significantly different before and after the intervention. However, the mean score of DHI decreased significantly after the intervention (P<0.001). Conclusion: Intratympanic dexamethasone/hyaluronic acid had a positive effect on the vertigo of the investigated patients without any significant improvement in hearing impairment and tinnitus in the short term. © 2019 Mashhad University of Medical Sciences. Mycosis Fungoides in Iranian Population: An Epidemiological and Clinicopathological Study
Background. Mycosis fungoides (MF) is the most common subtype of cutaneous T-cell lymphoma. Extensive studies on Iranian MF patients are absent. The present study aimed to produce updated clinical information on Iranian MF patients. Methods. This was a retrospective, descriptive, single-center study, including all cases of MF seen in the Department of Dermatology, University Hospital of Isfahan, Iran, between 2003 and 2013. Data systematically recorded for each patient included clinical, biological, histological, and molecular findings. Results. Eighty-six patients with clinical and histologic diagnosis of MF were included in the study. Thirty-nine patients (45.3%) were male. Female predominance was observed in patients (male : female ratio is 1 : 1.2). Patients were between 7 and 84 years of age (median: 41). The interval from disease onset to diagnosis ranged from 0 to 55 years (median: 1 year). Eighteen cases (20.9%) had unusual variants of MF. The most common types included hypopigmented and poikilodermatous MF. Childhood cases of MF constituted 5.8% (5/86) of all patients. The early stages were seen in 82 cases (95.34%). Conclusion. The major differences in epidemiologic characteristics of MF in Iran are the lack of male predominance and the lower age of patients at the time of diagnosis
