    Impact of the COVID-19 pandemic on mental health care and people with mental health conditions in Ethiopia:the MASC mixed-methods study

    BACKGROUND: The COVID-19 pandemic has had far-reaching effects on the mental health of populations around the world, but there has been limited focus on the impact on people with existing mental health conditions in low-income countries. The aim of this study was to examine impact of the pandemic on mental health care and people with mental health conditions in Ethiopia.METHODS: A convergent mixed methods study was conducted. We systematically mapped information from publicly available reports on impacts of the pandemic on mental health care. Monthly service utilisation data were obtained from Amanuel Mental Specialised Hospital, the main psychiatric hospital, and analysed using segmented Poisson regression (2019 vs. 2020). In-depth interviews were conducted with 16 purposively selected key informants. Framework analysis was used for qualitative data. Findings from each data source were integrated.RESULTS: In the early stages of the pandemic, participants indicated a minimal response towards the mental health aspects of COVID-19. Mental health-related stigma and discrimination was evident. Scarce mental health service settings were diverted to become COVID-19 treatment centres. Mental health care became narrowly biomedical with poorer quality of care due to infrequent follow-up. Households of people with pre-existing mental health conditions in the community reported worsening poverty and decreased access to care due to restricted movement, decreased availability and fear. Lack of reliable medication supplies increased relapse and the chance of becoming chained at home, abandoned or homeless. Caregiver burden was exacerbated. Within mental health facilities, prisons and residential units, infection control procedures did not adequately safeguard those with mental health conditions. Meanwhile, the needs of people with mental health conditions in COVID-19 quarantine and treatment facilities were systematically neglected. Only late in the day were integrated services developed to address both physical and mental health needs.CONCLUSIONS: The COVID-19 pandemic had substantial negative impacts on the lives of people with mental health conditions in Ethiopia. Future emergency response should prioritise the human rights, health, social and economic needs of people with mental health conditions. Integration of mental and physical health care would both expand access to care and increase resilience of the mental health system

Although, death rates from pandemics dropped globally by about 0.8 % per year, all the way through the 20th century, the number of new infectious diseases like Sars, HIV and Covid-19 increased by nearly fourfold over the past century. In Africa, there were reported a total of 4,522,489 confirmed COVID-19 cases and 119,816 death, as of 23 April 2021. The pandemic impacted seriously on the economic and social sectors in almost all African countries. It is threatening to push up to 58 m people into extreme poverty. However, apart from the African poor, the Covid pandemic also affects the growing African middle class, i.e. about 170 million out of Africa's 1.3 billion people currently classified as middle class. Nearly eight million of may be thrust into poverty because of the coronavirus and its economic aftermath. This setback will be felt for decades to come. Moreover, in recent African History also other infectouse diseases like the 1896-1906 Congo Basin Trypanosomiasis with a death-toll of over 500.000 as well as the 1900-1920 Uganda African trypanosomiasis epidemic with 200,000-300,000 death had tremendous negative impact on Africa's societies and economies. Actually, other pandemics, like Yellow Fever, Cholera, Meningitis and Measles - not to mention Malaria - contributed to long-lasting economic downturns and seriously affect the social wellbeing for decades.Im Laufe der Geschichte hat nichts mehr Menschen getötet als Infektionskrankheiten. Obwohl die Sterblichkeitsrate durch Pandemien im Laufe des 20. Jahrhunderts weltweit um etwa 0,8% pro Jahr gesunken ist, hat sich die Zahl der neuen Infektionskrankheiten wie Sars, HIV und Covid-19 im vergangenen Jahrhundert fast vervierfacht. In Afrika wurden zum 23. April 2021 insgesamt 4.522.489 bestätigte COVID-19-Fälle und 119.816 Todesfälle gemeldet. Die Pandemie hatte schwerwiegende Auswirkungen auf den wirtschaftlichen und sozialen Sektor in fast allen afrikanischen Ländern. Sie droht, bis zu 58 Millionen Menschen in extreme Armut zu treiben. Abgesehen von den afrikanischen Armen betrifft die Covid-Pandemie jedoch auch die wachsende afrikanische Mittelschicht, d. h. etwa 170 Millionen der 1,3 Milliarden Menschen in Afrika, die derzeit als Mittelschicht eingestuft sind. Fast acht Millionen von ihnen könnten aufgrund des Coronavirus und seiner wirtschaftlichen Folgen in Armut geraten. Dieser Rückschlag wird noch Jahrzehnte zu spüren sein. Darüber hinaus hatten in der jüngeren afrikanischen Geschichte auch andere Infektionskrankheiten wie die Trypanosomiasis (Schlafkrankheit) im Kongobecken von 1896-1906 mit einer Zahl von über 500.000 Todesopfern sowie die Trypanosomiasis-Epidemie in Uganda von 1900-1920 mit 200.000-300.000 Todesfällen enorme negative Auswirkungen auf die afrikanischen Gesellschaften und Volkswirtschaften. Tatsächlich haben andere Pandemien wie Gelbfieber, Cholera, Meningitis und Masern - ganz zu schweigen von Malaria - zu lang anhaltenden wirtschaftlichen Abschwüngen beigetragen und das soziale Wohlbefinden über Jahrzehnte hinweg ernsthaft beeinträchtigt.Au cours de l’histoire, rien n’a tué plus d’êtres humains que les maladies infectieuses et la fièvre hémorragique. Bien que les taux de mortalité dus aux pandémies aient chuté de près de 1% par an dans le monde, environ 0,8% par an, tout au long du XXe siècle, le nombre de nouvelles maladies infectieuses comme le Sars, le VIH et le Covid-19 a presque quadruplé par rapport au passé. En Afrique, on a signalé un total de 4 522 489 cas confirmés de COVID-19 et 119 816 décès, au 23 avril 2021. La pandémie a eu de graves répercussions sur les secteurs économique et social dans presque tous les pays africains. Il menace de pousser jusqu'à 58 millions de personnes dans l'extrême pauvreté. Cependant, outre les Africains pauvres, la pandémie de Covid affecte également la classe moyenne africaine en pleine croissance, c'est-à-dire environ 170 millions sur les 1,3 milliard d'africains actuellement classés dans la classe moyenne. Près de huit millions d'entre eux pourraient être plongés dans la pauvreté à cause du coronavirus et de ses conséquences économiques. Ce revers se fera sentir pendant des décennies.     African Handbook of Climate Change Adaptation

    This open access book discusses current thinking and presents the main issues and challenges associated with climate change in Africa. It introduces evidences from studies and projects which show how climate change adaptation is being - and may continue to be successfully implemented in African countries. Thanks to its scope and wide range of themes surrounding climate change, the ambition is that this book will be a lead publication on the topic, which may be regularly updated and hence capture further works. Climate change is a major global challenge. However, some geographical regions are more severly affected than others. One of these regions is the African continent. Due to a combination of unfavourable socio-economic and meteorological conditions, African countries are particularly vulnerable to climate change and its impacts. The recently released IPCC special report "Global Warming of 1.5º C" outlines the fact that keeping global warming by the level of 1.5º C is possible, but also suggested that an increase by 2º C could lead to crises with crops (agriculture fed by rain could drop by 50% in some African countries by 2020) and livestock production, could damage water supplies and pose an additonal threat to coastal areas. The 5th Assessment Report produced by IPCC predicts that wheat may disappear from Africa by 2080, and that maize— a staple—will fall significantly in southern Africa. Also, arid and semi-arid lands are likely to increase by up to 8%, with severe ramifications for livelihoods, poverty eradication and meeting the SDGs. Pursuing appropriate adaptation strategies is thus vital, in order to address the current and future challenges posed by a changing climate. It is against this background that the "African Handbook of Climate Change Adaptation" is being published. It contains papers prepared by scholars, representatives from social movements, practitioners and members of governmental agencies, undertaking research and/or executing climate change projects in Africa, and working with communities across the African continent. Encompassing over 100 contribtions from across Africa, it is the most comprehensive publication on climate change adaptation in Africa ever produced

    Plants and Plant Products in Local Markets Within Benin City and Environs

    AbstractThe vulnerability of agriculture systems in Africa to climate change is directly and indirectly affecting the availability and diversity of plants and plant products available in local markets. In this chapter, markets in Benin City and environs were assessed to document the availability of plants and plant products. Markets were grouped into urban, suburban, and rural with each group having four markets. Majority of the plant and plant product vendors were women and 88 plant species belonging to 42 families were found. Their scientific and common names were documented as well as the parts of the plant and associated products available in the markets. Most of the plant and plant products found in local markets belong to major plant families. Urban markets had the highest diversity of plants and plant products. Three categories of plants and plant products were documented. Around 67% of the plants and plant products were categorized as whole plant/plant parts, 28% as processed plant parts, while 5% as reprocessed plant/plant parts. It was revealed that 86% of these plants are used as foods, 11% are for medicinal purposes, while 3% is used for other purposes. About 35% of plants and plant products across the markets were fruits, which is an indication that city and environs are a rich source of fruits. The local knowledge and practices associated with the plants and plant products can contribute towards formulating a strategic response for climate change impacts on agriculture, gender, poverty, food security, and plant diversity

    AbstractThe lack of affordable, clean, and reliable energy in Africa's rural areas forces people to resort to poor quality energy source, which is detrimental to the people's health and prevents the economic development of communities. Moreover, access to safe water and food security are concerns closely linked to health issues and children malnourishment. Recent climate change due to global warming has worsened the already critical situation.Electricity is well known to be an enabler of development as it allows the use of modern devices thus enabling the development of not only income-generating activities but also water pumping and food processing and conservation that can promote socioeconomic growth. However, all of this is difficult to achieve due to the lack of investors, local skills, awareness by the community, and often also government regulations.All the above mentioned barriers to the uptake of electricity in rural Kenya could be solved by the coordinated effort of government, private sector, and academia, also referred to as Triple Helix, in which each entity may partially take the other's role. This chapter discretizes the above and shows how a specific county (Marsabit) has benefited from this triple intervention. Existing government policies and actions and programs led by nongovernmental organizations (NGOs) and international agencies are reviewed, highlighting the current interconnection and gaps in promoting integrated actions toward climate change adaptation and energy access
