Role of diffusion-weighted magnetic resonance (MR) imaging in differentiation between graves' disease and painless thyroiditis
Background: To assess the role of diffusion-weighted MR imaging in differentiation between Graves' disease and painless thyroiditis. Material/Methods: A prospective study was conducted among 37 consecutive patients with untreated thyrotoxicosis (25 female and 12 male; mean age of 44 years) and 15 age- and sex-matched controls. Diffusion-weighted MR imaging of the thyroid gland was performed in patients and controls. The apparent diffusion coefficient (ADC) value of the thyroid gland was calculated and correlated with Tc-99m uptake and thyroid function tests of the patients. Results: There was a significant difference in the ADC value of the thyroid gland between patients and the control group (P=0.001). The mean ADC value of the thyroid gland in Graves' disease was 2.03±0.28×10-3 mm2/sec, and in patients with painless thyroiditis 1.46±0.22×10-3 mm2/sec, respectively. There was a significant difference in the ADC values between Graves' disease and painless thyroiditis (P=0.001). When the ADC value of 1.45×10-3 mm2/sec was used as a threshold value for differentiating Graves' disease from painless thyroiditis, the best result was obtained with area under the curve of 0.934, accuracy of 83.8%, sensitivity of 95.8%, and specificity of 61.5%. The mean ADC value of the thyroid gland in patients positively correlated with serum TRAb and Tc-99m uptake (r=0.57, P=0.001 and r=0.74, P=0.001, respectively). Conclusions: We concluded that ADC values of the thyroid gland can be used to differentiate Graves' disease from painless thyroiditis in patients with untreated thyrotoxicosis
The association of TSH-receptor antibody with the clinical and laboratory parameters in patients with newly diagnosed Graves' hyperthyroidism: experience from a tertiary referral center including a large number of patients with TSH-receptor antibody-negative patients with Graves' hyperthyroidism
Introduction: Although the TSH-receptor antibody (TRAb) plays a central role in the pathogenesis of Graves' disease (GD), the association between TRAb at first diagnosis and clinical and laboratory parameters is not well known. On the other hand, a minority of patients with GD may be TRAb negative, and there is a lack of adequate evidence to demonstrate the clinical and laboratory characteristics of these patients. Therefore, we aimed to investigate the association of TRAb at the initial diagnosis of GD with the clinical and laboratory parameters in a large number of patients with GD and to compare the clinical and laboratory parameters between patients with high TRAb levels and TRAb-negative patients. Material and methods: This study included 440 patients [326 (74%) female, 114 (26%) male]. All patients were classified according to gender, age, smoking habit, and TRAb levels. Results: TRAb levels were significantly higher in male compared to female patients and in smokers compared to non-smokers. Smoking male patients had the highest TRAb levels. In regression analysis, goiter size, male gender, cigarette smoking, Graves' orbitopathy, fT3, and anti-TPO antibody levels were independently associated with high TRAb levels, while age at diagnosis and fT4 levels were not independently associated with high TRAb levels. TRAb-negative GD was diagnosed in 80 (18%) patients. TRA-negative patients had markedly less severe clinical and laboratory hyperthyroidism compared to patients with high TRAb levels. Moreover, the smoking habit was significantly lower in patients with TRAb-negative GD. Conclusions: According to our study results, TRAb levels at the initial diagnosis of GD are differently associated with clinical and laboratory parameters. Male patients and smoking patients with GD tended to have markedly higher TRAb levels and more severe clinical hyperthyroidism. Therefore, besides other contributing factors, male gender and smoking may affect TRAb levels and consequently the severity of hyperthyroidism in patients with GD. Furthermore, male gender and smoking may have a synergistic effect on TRAb levels and consequently on the severity of hyperthyroidism in patients with GD

Diagnosis, treatment and long-term consequences of hyperthyroidism: use of existing data to generate new knowledge
Hyperthyroidism is a common endocrine disorder with multiple aetiologies, manifestations and potential therapies. This thesis explores the challenges relating to the diagnosis, treatment and long-term consequences of thyrotoxicosis in real-world outpatient and inpatient settings. We performed a number of epidemiological studies analysing data from large, detailed, routinely collected data sources. We confirmed that classical manifestations of hyperthyroidism are significantly less prevalent in older patients and established that newly diagnosed thyroid dysfunction is rare in hospitalised subjects despite high volume thyroid function testing in this setting where we found a high proportion of abnormal thyroid tests in those with pre-existing thyroid dysfunction. We determined that thionamides are effective in a half of subjects treated with a prolonged course and that appropriate patient selection improves success rates. We established that treatment of hyperthyroidism with radioactive iodine results in more weight gain than antithyroid drugs and that hyperthyroidism in hospitalised patients is associated with longer hospital stays, higher frequency of admissions and increased mortality. In conclusion, this thesis provides important new insights into the diagnosis and treatment of hyperthyroidism and highlights that correct evaluation and management of patients may minimise the long-term consequences associated with this common disorder
