Integration of health and social care in a regional area of Scotland: exploring the needs and experiences of service users and their families.

Abstract

AIM: - To explore the experiences of service users and their families accessing integrated health and social care services to meet their health and wellbeing needs. OBJECTIVES: 1. To identify the experiences of service users and their families who access integrated health and social care services within Grampian. 2. To describe the extent to which the preferences of services users and their families are met in achieving their health and wellbeing needs. 3. To evaluate the utility of integrated health and social care services in meeting the health and wellbeing needs of service users.
