<p>*Contingency tables assessing the correlation between the binary variables used to construct the knowledge indexes are included in supplementary file <a href="http://www.plosone.org/article/info:doi/10.1371/journal.pone.0130845#pone.0130845.s001" target="_blank">S1 Appendix</a>.</p><p>**Providers were not penalized for slight variations in the interpretation of age cut-offs.
