Determinantes organizacionales del ambiente en el hospital: análisis empírica con microdatos sobre el cumplimiento por el modelo de acreditación hospitalar

Abstract

O aumento dos gastos nas unidades hospitalares (UHs) tem motivado a busca de soluções gerenciais para melhorar o ambiente hospitalar. Este estudo testa a associação entre a gerência do ambiente hospitalar e as seguintes funções de organização da Acreditação Hospitalar (AH): o treinamento; a educação do paciente e do familiar; a governabilidade; e a gerência da informação. A amostra de conveniência é composta de 47 relatórios da AH. Foram usados o modelo de regressão múltipla e a Análise Fatorial para avaliar a influência daquelas funções de organização sobre a função Gerência do Ambiente Hospitalar. Os testes da hipótese revelaram associações positivas e significativas entre as funções de organização selecionadas, o tipo de propriedade da UH e a gerência do ambiente hospitalar. Como contribuição teórica, esclarece as múltiplas dimensões envolvidas no conceito de ambiente hospitalar, que são responsáveis pela sua influência na busca da qualidade nas UHs. Como contribuição metodológica, utiliza dados de relatórios da AH, pouco explorados na pesquisa sobre economia da gestão. Como contribuição gerencial, identificou fatores influentes no desempenho da Gerência do Ambiente Hospitalar.Increased spending on hospital units (HUs) has motivated the search for managerial solutions to improve the hospital environment. This study tests the association between management of the hospital environment and the following organizational functions defined by the Hospital Accreditation (HA) model: training, education of the patient and his family; governance, and information management. The convenience sample is composed by 47 reports of HA. Statistical analysis was performed with the help of multiple regression models and factor analysis. The hypothesis tests revealed significant positive associations between the organizational functions selected, the property of the HU and the hospital environment. As a theoretical contribution, the paper clarifies the multiple dimensions involved in the concept of hospital environment, which are responsible for its influence in the pursuit of quality in HUs. As a methodological contribution, the statistical model uses data from reports of HA, little explored in the research about economics of management. As a managerial contribution, the paper identifies influential factors in the performance of the Management of the Hospital Environment
