ao longo do tempo. Atualmente existem vários estudos dedicados à degradação
enzimática, mas poucos exploram as estratégias de prevenção da degradação hidrolítica.
A degradação hidrolítica é uma das principais responsáveis pela deterioração da camada híbrida.
Objetivo: Avaliar tratamentos que pretendem prevenir a degradação hidrolítica na camada híbrida da dentina, comparando os resultados imediatos e a longo prazo da
resistência adesiva das diferentes intervenções.
Materiais e Métodos: A pesquisa da revisão sistemática foi realizada através de 5 bases
de dados: PubMed/Medline; Scopus; Web of Science; Open Grey e Mednar, tendo sido
selecionadas referências dos últimos 6 anos. Avaliou-se o risco de viés nos 11 estudos
incluídos na revisão sistemática e foram selecionados 9 estudos para a meta-análise em
rede.
Resultados: A técnica etanol wet bonding (EWB), quando utilizada em combinação com
o inibidor enzimático riboflavina, demonstrou melhores resultados na preservação da
resistência adesiva que os outros tratamentos analisados (p<0.001). A estratégia etch-andrinse mostrou ser mais eficaz na manutenção da interface adesiva quando realizada
juntamente com DMSO a 50% (p=0.014).
Conclusão: A utilização de estratégias de desidratação influenciou positivamente os
resultados da resistência adesiva a longo prazo. A combinação de EWB com inibidores
enzimáticos como a riboflavina parece ser a estratégia que tem mais capacidade de
estabilizar a interface adesiva. É essencial a realização de mais estudos para uma possível futura aplicação clínica.Introduction: Adhesive restorations are prone to degradation within the hybrid layer
(HL), which is persistently described as the weakest link. A lot of research has been
dedicated to enzymatic breakdown; however, few studies have focused on strategies to
prevent hydrolytic degradation, which is still one of the major reasons of HL
deterioration.
Objective: To evaluate treatments that deem to prevent hydrolysis within the hybrid
layer, by comparing immediate vs. long-term bond strength results of different
interventions.
Materials and Methods: The systematic review research was carried out through 5
databases: Pubmed/Medline; Scopus; Web of Science; Open Gray and Mednar.
References were selected from the last 6 years. The risk of bias was assessed in the 11
studies included in the systematic review and 9 studies were selected for the network
meta-analysis.
Results: The ethanol wet bonding technique (EWB) when used in combination with the
enzymatic inhibitors riboflavin (p <0.001), showed better results compared to other
treatments. DMSO with 50% concentration applied together with the etch-and-rinse
strategy has also shown to be statistically different from the control group (p = 0.014).
Conclusion: The use of dehydration strategies positively influenced the results of longterm bond strength. The combination of EWB with enzyme inhibitors such as riboflavin seems to be the strategy that has the best ability to stabilize the adhesive interface. Further studies are essential for possible future clinical applications
