Chronic kidney disease (CKD) is a disease with a prevalence that is always increasing
every year and hemodialysis needs to be done at the final stage Routine hemodialysis requires
the installation of a permanent AV Shunt by performing high-cost surgery so it must be
monitored systematically. The JKN system paid by BPJS to hospitals uses the INA-CBG
system with cost components consisting of the costs of medicines, medical devices, operating
room rents, doctor's services, laboratories, nursing actions, and administration which are
counted integrated in packages covered by the health insurance. The purpose of this study
was to analyze the differences in real costs and the costs of INA-CBG's at the A. Yani
Islamic Hospital with a non-retrospective research design. Medical record data and BPJS
claim data for AV Shunt installation operations can be used to compare differences in real
hospital costs and the cost of INA CBG package rates with statistical analysis. The results of
this study were 188 patients. The average total cost of direct medical real BPJS participants in
class I is Rp. 4.689.987 / patient, class II Rp. 3.029.017 / patient and class III Rp. 2.648.601 /
patient. The average total cost of INA-CBG’s, class I is Rp. 10.673.120, class II Rp.8.866.721 and class III Rp. 8.048.999. Analysis for class I with independent t-test obtained P value (0,000). In patients in class II and class III with the Mann Whitney U Test with a P value (0,000). So that in classes I, II, and III there is a significant difference between the realcost and INA-CBG package rates.
