Síndrome Felty

Abstract

A clinic case of a 62-year-old patient with a history of 14-year evolution rheumatoid arthritis with empirical treatment, who went to the emergency department for fever associated with abdominal pain and constipation. On physical examination, no fever, no signs of systemic inflammatory response syndrome, vital signs within normal parameters, abdominal palpation, it is found enlarged spleen, confirmed by ultrasound and CT of the abdomen. Hemogram with decreased leukocytes and neutrophils, high rheumatoid factor, bone marrow aspirated with hypercellularity for her age were considered as a diagnosis of Felty's syndrome. This clinic case is presented because of the low incidence of this syndrome in the population since about 1% of patients with rheumatoid arthritis have it, and if an early management is started, the progression of the disease can be avoided.Se presenta el caso clínico de una paciente de 62 años con antecedente de Artritis Reumatoide de 14 años de evolución con tratamiento empírico, quien consultó a urgencias por fiebre, asociada a dolor abdominal y estreñimiento. Al examen físico, afebril, sin signos de Síndrome de Respuesta Inflamatoria Sistémica, signos vitales dentro de parámetros normales, a la palpación abdominal, se encuentra bazo aumentado de tamaño, confirmado por ecografía y TAC de abdomen. Hemograma con leucocitos y neutrófilos disminuidos, factor reumatoideo elevado, aspirado de médula ósea con hipercelularidad para la edad se considera diagnóstico de síndrome de Felty. Se trae este caso clínico debido a la baja incidencia de este síndrome en la población, pues aproximadamente el 1 % de los pacientes con Artritis Reumatoide lo presentan, y si se inicia un manejo temprano se puede evitar la progresión de la patología
