Selective laser trabeculoplasty vs Argon laser trabeculoplasty in open angle glaucoma.



Purpose: To compare the effectiveness in lowering the intraocular pressure and the anterior chamber reaction after selective laser trabeculoplasty (SLT) or argon laser trabeculosplasty (ALT). Methods: Thirty patients aged from 42 to 73 were randomised to treatment with either SLT or ALT. SLT was performed by a 532 nm Nd:YAG laser and ALT by a 532 nm argon laser. Before laser trabeculoplasty we perfomed an evaluation of anterior chamber cellularity with a flare-cell meter (Kowa 500). ALT was performed using 50 applications of 50 \ub5m, 0.1 msec. duration and a power ranged from 450 mW to 620 mW (mean 513.333\ub148.990). SLT was performed using 50 spots with a size of 400 \ub5m, pulse duration of 3 ns and a power ranged from 0.400 to 0.700 (mean 0.572\ub10.115). One day after we performed a flare cell meter evaluation. Intraocular pressure was evaluated one day, one week, one and three months after. Results We treated 24 eyes in each group. Baseline the IOP in the SLT group and in ALT group was 23.263\ub12.600 mm/Hg vs 22.632\ub12.967 mm/Hg (p<0.459) the day after treatment we found 18.705\ub12.800 mm/Hg vs 19.633\ub12.780 mm/Hg (p<0.430). One month later 16.713\ub12.100 mm/Hg vs 17.650\ub12.600 mm/Hg (p<0.390) and after three months 14.842\ub12.167 mm/Hg vs 16.053\ub12.223 mm/Hg (p<0.317). The baseline anterior chamber flare-cell in SLT and ALT was 4.600\ub11.405 photons/ms vs 6.311\ub13.416 photons/ms (p<0.169) and the day after treatment was 5.960\ub10.983 photons/ms vs 13.289\ub12.153 photons/ms (p<0.027). Conclusions: SLT appears to be statistically equivalent to ALT in lowering IOP during the first three months after treatment even if the SLT patients have a greater drop in IOP. We found a lower anterior chamber reaction one day after SLT
