Heart of Ohio Family Health

Abstract

IMPACT. 1: Heart of Ohio Family Health provided care to over 7,500 patients in 2016. We serve diverse communities, with over one-third of our patients best served in a language other than English. Furthermore, over half of our patients fall below 100% of the poverty guideline. We aim to provide a variety of needed services to patients within our centers through our staff and partnerships with other organizations. -- 2. The Ohio State University's College of Pharmacy provides clinical support and onsite pharmacy services at both Capital Park and Whitehall Family Health Centers. In 2016 the team of clinical pharmacists, pharmacy residents, and student pharmacists provided care to almost 700 patients in the areas of diabetes, prediabetes, lipids, hypertension, anticoagulation, tobacco cessation, and comprehensive medication reviews. -- 3. Other Ohio State partnerships include coaching by OSU nurse practitioner students for Spanish-speaking diabetes patients through the Integrated Second Language Learning Program and The James Mobile Mammography Unit providing over 300 mammograms this year onsite for our patients.
