Intubation of cystic duct in the surgical management of billiart tract diseases.

Abstract

THE ORDER TO STUDY THE OPERATIVE TREATMENT OF THE COMMON BILE DUCT LITHIASIS AND THE COMPARISON OF THE BILIARY TREE DRAINAGE* ,2279 PATIENTS(945 AND 1336) WHO UNDERWENT CHOLOCYSTECTOMY FROM 1982 TO 1989 WERE EXAMINED.FROM THESE,480(21,1% OF THE TOTAL) WERE IN NEED FOR BILIARY TREE EXPLOTATION.IN 394 A T-TUBE IN COMMON BILE DUCT WAS USED (GROUP B1,WITHOUT CHOLEDOCTOTOMY,EXPLORATION THROUGH THE CYSTIC DUCT) AND IN THE REST 17 A NELATON TUBE IN CYSTIC DUCT AFTER CHOLEDOCTOTOMY WAS USED(B2).HOSPITALIZATION TIME(IN DAYS) WAS SIGNIFICANTLY LESS IN PATIENTS OF THE GROUP B1(7,8=1,6) AND OF THE B2(9,1=2,7) THAN THAT IN THE PATIENTS OF THE GROUP A(13,4=4,8).MAJOR COMPLICATIONS(BILIARY PERITONITIS,INFRAHEPATIC COLLECTION)WERE NOTICED ONLY IN PATIENTS OF GROUP A.IN CONCLUSION,THE DRAINAGE OF THE BILIARY TREE WITH A NELATON TUBE IN CYSTIC DUCT IS A TRIED AND SAFE METHOD OF DECOMPRESSION EVEN AFTER CHOLEDOCTOTOMY AND MAY APPLY IN THE MODERN LAPAROSCOPIC METHOD OF CHOLECYSTECTOMY WHEN EVER SUCH A DRAINAGE MAY BE NEEDED. *EITHER WITH A T-TUBE IN COMMON BILE DUCT OR A NELATON TUBE IN CYSTIC DUCT.
