RELATIONSHIP BETWEEN URINARY PROSTAGLANDIN E. LEVELS AND URINARY SODIUM EXCRETION OR BLOOD PRESSURE IN PATIENTS WITH RENAL DISEASE.

Abstract

URINARY PGE LEVELS WERE DETERMINED BY A BIOASSAY METHOD FOLLOWING EXTRACTION, PURIFICATION AND THIN - LAYER CHROMATOGRAPHIC SEPARATION IN PATIENTS WITH RENAL DISEASE. THE BIOASSAY DETERMINATION WAS PERFORMED IN CENTRE DE RECHERCHESSUR L'HYPERTENSION ARTERIELLE, I.N.S.E.R.M.U - 28, PARIS BY DR. N. PAPANICOLAOU. MEAN URINARY PGES WERE SIGNIFICANTLY LOWER IN PATIENTS WITH GLOMERULAR DISEASE, THAN IN PATIENTS WITH DISEASE AFFECTING PRIMARILY THE RENAL MEDULLA (SUCH AS POLYCYSTIC DISEASE, INTERSTITIAL NEPHROPATHY, CHRONIC PYELONEPHRITIS, ACUTE TUBULAR NECROSIS ETC). THERE WAS A SIGNIFICANT POSITIVE CORRELATION BETWEEN URINARY PGE LEVELS AND URINARY SODIUM EXCRETION IN ALL PATIENTS AS WELL AS IN THE PATIENTS WITH RENAL MEDULLARY DISEASE. URINARY PGES WERE NEGATIVELY CORRELATED TO MEAN BLOOD PRESSURE IN ALL PATIENTS AND IN THE GLOMERULAR DISEASE PATIENT GROUP. NO SIGNIFICANT CORRELATION EXISTED BETWEEN PGES AND SERUM CREATININE LEVELS. IT IS CONCLUDED THAT: 1. RENAL PGE SYNTHESIS AND/OR RELEASE VARIES AMONG PATIENTS WITH RENAL DISEASE. 2. RENAL PGE SYNTHESIS AND/OR RELEASE DEPENDS ON THE SITE OF THE PRIMARY LESION WITHIN THE KIDNEY RATHER THAN ON THE SEVERITY OF RENAL DISEASE. 3. RENAL PGE SYNTHESIS AND/OR RELEASE IN RENAL DISEASE IS RELATED TO URINARY SODIUM ESCRETION AND BLOOD PRESSURE LEVELS. 4. IN VIEW OF THE CONTROVERSY ON THE RELATIONSHIP BETWEEN PGES AND SODIUM, IT IS DIFFICULT TO INTERPRETE THE POSITIVE CORRELATION BETWEEN URINARY PGES AND NATRIURESIS. 5. THE DECREASE IN RENAL PGE SYNTHESIS AND/OR RELEASE IN CASES OF ARTERIAL HYPERTENSION MIGHT BE (ABSTRACT TRUNCATED)
