Η ΑΙΤΙΟΛΟΓΙΑ ΤΗΣ ΒΟΥΒΩΝΟΚΗΛΗΣ ΤΩΝ ΕΝΗΛΙΚΩΝ ΚΑΙ Η ΘΕΡΑΠΕΥΤΙΚΗ ΑΝΤΙΜΕΤΩΠΙΣΙΣ ΑΥΤΗΣ ΔΙΑ ΤΗΣ ΠΛΑΣΤΙΚΗΣ MC VAY

Abstract

A STUDY OF 201 PATIENTS, 184 MALE AND 17 FEMALE, OVER THE AGE OF 40 WITH INGUINAL HERNIA IS PRESENTED. THEY ALL HAD A REPAIR DONE, AS DESCRIBED BY MC VAY, AT THE 2ND SURGICAL UNIT EVANGELISMOS MEDICAL CENTER FROM FEBRUARY 1972 UNTIL MAY 1977. DATA HAVE BEEN COLLECTED ON THE AETIOLOGY OF THE HERNIA ON ALL PATIENTS, TWO YEARS RETROSPECTIVELY AND THE REMAINING YEARS PROSPECTIVELY. 231 OPERATIONS WERE PERFORMED ON THE 201 PATIENTS FOR REPAIR OF THE POSTERIOR WALL OF THE INGUINAL CANAL. THE RATE OF POSTOPERATIVE COMPLICATIONS WAS 9,5% WHILE THAT OF MORTALITY WAS 1%. OF THE 201 PATIENTS OPERATED UPON, 151 WERE FOLLOWED UPFOR A PERIOD OF 2,5 TO 7,5 YEARS (128 PATIENTS WITH UNILATERAL AND 23 WITH BILATERAL HERNIAS). RECURRENCES OCCURED BETWEEN THE 18TH AND 24TH MONTH AFTER THE REPAIR AT A RATE 3,5%. IT WAS FOUND THAT THE PROBABILITY OF PATIENTS WITH UNILATERAL HERNIA DEVELOPING ANOTHER ONE ON THE OPPOSITE SIDE WAS THREE TO FIVE TIMES HIGHER THAN THAT OF HEALTHY PEOPLE DEVELOPING A HERNIA. THE COLLECTED DATA CONCERNING THE AETIOLOGY OF INGUINAL HERNIA IN ADULTS SUPPORT THE BIOLOGICAL VIEWPOINT. THERE WAS A DIRECT PROPORTIONAL RELATIONSHIP BETWEEN THE AGE OFTHE PATIENTS AND THE FACTORS CONTRIBUTING TO THE ONSET OF THE DISEASE. IT SEEMS THAT THE PRE - EXISTING ANATOMICAL DEFECTS BECOME APPARENT WHEN THE METABOLICAL DISTURBANCES OF THE COLLAGEN OCCUR AND DETERMINE THE POINT OF PROTRUSION OF THE HERNIAL SAC.
