THE CONTRIBUTION OF THE LEFT COLON IN ESOPHAGOPLASTY

Abstract

THE PRESENT STUDY HAS BEEN MADE POSSIBLE AFTER PROFESSOR P. CHRYSSOSPATHISES ADVICE, WHO SO KINDLY, GAVE US THE MATERIAL OF HIS WIDE EXPERIENCE TO BE ANALYSED AND STUDIED IN DETAIL. IT DEALS WITH THE POSSIBILITY OF USING THE LEFT COLON IN ESOPHAGOPLASTIES AND A SPECIAL INVESTIGATION HAS BEEN MADE ON THE BEHAVIOR OF THE COLONIC GRAFT IF THAT IS PLACED ISO OR ANTIPERISTALTICALLY. THE EXAMINED MATERIAL CONSISTS OF 160 CASES OF ESOPHAGETOPLASTIES OPERATED UPON FROMTHE YEAR 1954 UP TO 1974, IN THE 1ST SURGICAL PROFESSORIAL UNIT OF THE ATHENS UNIVERSITY OR ELSEWHERE. IN ALL THOSE CASES THE LEFT COLON WAS USED, IN 113OF THEM ISOPERISTALTICALLY AND IN 47 ANTIPERISTALTICALLY. OUR CONCLUSIONS, EXTRACTED FROM THE ABOVE MATERIAL ARE IN ACCORDANCE WITH THOSE FOUND IN THE INTERNATIONAL LITERATURE AND ARE SUMMARIZED AS FOLLOWS: 1) THE LEFT COLON IS CONSIDERED SUPERIOR IN COMPARISON TO THE RIGHT IN THIS KIND OF OPERATIONS, BECAUSE IT CAN BE USED IN EVERY ONE OF THEM. THE RIGHT COLON IS USED ONLY IF THE LENGTH OF THE RIGHT COLIC ARTERY PERMITS SO, A RESTRICTING FACTOR WHICH MUST NEVER BE OVERLOOKED. 2) THE LEFT COLON CAN BE PLACED ISO OR ANTIPERISTALTICALLY WITH EASE IN COMPARISON TO THE RIGHT ONE WHICH MUST BE ALWAYS PLACED ISOPERISTALTICALLY. THIS IS MAINLY DUE TO THE FACT THAT THE LEFT COLONIC VESSELS ARE MORE STABLE WITH NO VARIATION AND THE MARGINAL ARTERY ALWAYS WELL FORMED AND PRESENT.
