Abstract

INTRACARDIAC MICROMANOMETRIC CATHETERIZATION HAS BEEN UNDERTAKEN IN A LARGE NUMBER OF HOSRITALIZED PATIENTS SELECTED FROM CONGENITAL OR AQUIRED GROUPS OF HEART DISEASES.THIS PROCEDURE COULD BE CONDUCTED BY A SPECIALISED PHYSICIAN WITH EXPERIENCE IN CONCERN OF TECHNIQUE AND OF APPRECIATION OF THE DERIVED CURVES.ANALYSIS OF TH FINDINGS IS DERIVED FROM EXPERIENCE RATHER THAN FROM THE THEORETICAL APPROACH TO CONCEPTIONS GOVERNING SOUND TRANSMISSION.WE HAVE EXAMINED OUR PATIENTS BY THE CONVENTIONAL METHODS PLUS THE MICROMANOMETRIC INVESTIGATIONAND COMPARING THE OBTAINED RESULTS OF THESE TWO WAYS OF EXAMINATION WE HAVE CONCLUDED AS FOLLOWING:-IN 39,3% OF THE CASES A FULL AGREEMENT WAS FOUND BETWEEN THE TWO GROUPS.-IN 10,7% OF THE CASES A PARTIAL CONCORDANCE WAS DETECTED.-IN 42,8% OF THE CASES A FULL OR PARTIAL DISCORDANCE WAS EVIDENT,AND -IN 7,1% OFTHE CASES THE MICROMANOMETRIC METHOD WAS MERELY PATHOGNOMIC.THIS METHOD HAS THE SAME VALUE AS THE ANGIOCARDIOGRAPHY IN UNCOVERING ARCHITECTURAL CARDIAC CHANGES.MICROMANOMETRIC INTRACARDIAC SOUND RECORDINGS APPEAR TO BE PARTICULARLY VALUABLE IN THE UNMASKING OF TRICUPSID INSUFICIENCY.THE USEFULNESS OF THE METHOD SHOULD BE STRESSED AGAIN IN DETERMINING THE TYPE OF PULMONIC STENOSIS I.E.VALVAR,SUBVALVAR,COMBINED AND FUNCTIONAL.LEFT HEART SOUNDS MAY BE PICKED UP WHEN THE MICROMANOMETRIC TIP IS LOCATED IN THE RIGHT HEART CHAMBERS.
