RESPIRATORY FUNCTION CHANGES AFTER LAPAROSCOPIC CHOLECYSTECTOMY

Abstract

THE POSTOPERATIVE CHANGES IN PULMONARY FUNCTION AFTER LAPAROSCOPIC AND OPEN CHOLECYSTECTOMY WERE EVALUATED IN THIS PROOPERATIVE STUDY IN ORDER TO DETERMINE WHETHER LAPAROSCOPIC CHOLECYSTECTOMY RESULTS IN LESS RESPIRATORY IMPAIRMENT. 30 PATIENTS UNDERGOING LAPAROSCOPIC AND 30 PATIENTS UNDERGOING OPEN CHOLECYSTECTOMY WERE STUDIED. PULMONARY FUNCTION TEST ARTERIAL BLOOD GAS ANALYSIS AND CHEST RADIOGRAPHS WERE OBTAINED PREOPERATIVELY AND ON THE FIRST POSTOPERATIVE DAY. POSTOPERATIVELY SIGNIFICANT REDUCTIONS IN FVC, FEV1 AND FEF 25-75% WERE OBSERVED IN BOTH GROUPS. THE REDUCTION OF FCV, FEV1 AND FEF 25-75% WERE GREATER AFTER OPEN THAN AFTER LAPAROSCOPIC CHOLECYSTECTOMY (34%, 41% AND 39% VERSUS 23%,25% AND 25% RESPECTIVELY). FRC DECREASED ONLY AFTER OPEN CHOLECYSTECTOMY (12%POSTOPERATIVELY). ATELECTASIS WAS LOWER (6,7% VERSUS 16,7%) AND ARTERIAL OXYGENATION BETTER AFTER LAPAROSCOPIC CHOLECYSTECTOMY. IN CONCLUSION PULMONARY FUNCTION IS LESS IMPAIRED AFTER LAPAROSCOPIC CHOLECYSTECTOMY COMPARED TO OPEN CHOLECYSTECTOMY THIS MAY RESULT IN A REDUCED INCIDENCE OF RESPIRATORY COMPLICATIONS AFTER LAPAROSCOPIC CHOLECYSTECTOMY.
