GASTRIC PARTITION IN THE SURGICAL TREATMENT OF CLINICALLY SEVERE OBESITY

Abstract

VERTICAL BANDED (MASON) GASTROPLASTY (VBG) AND VERTICAL RING (ECKHOUT) GASTROPLASTY (VRG), ARE THE CURRENTLY MOST WIDELY USED PROCEDURES FOR THE SURGICAL TREATMENT OF CLINICALLY SEVERE OBESITY. THE PRESENT STUDY DESCRIBES THE OUTCOME OF 40 PATIENTS WITH CLINICALLY SEVERE OBESITY WHO UNDERWENT VERTICAL BANDED ORRING GASTROPLASTY. AFTER AN EXTENSIVE PREOPERATIVE EVALUATION, 15 PATIENTS UNDERWENT VBG AND 25 PATIENTS VRG. DURING THE FOLLOW-UP PERIOD WHICH WAS EXTENDED IN THE 35 PATIENTS OF THE 40, TO 58 MONTHS, BOTH PROCEDURES PRODUCED A SATISFACTORY WEIGHT LOSS WITH NO DIFFERENCE IN THE SUCCESS RATE, AND NO LATE WEIGHT REGAIN. THE MORBIDITY IN THE EARLY AND LATE POSTOPERATIVE PERIOD WAS MINIMAL AND INSIGNIFICANT. WEIGHT REDUCTION AFTER VBG OR VRG WAS ASSOCIATED WITH DRAMATIC IMPROVEMENT OF COMMORBID CONDITIONS. IN CONCLUSION BOTH, VBG AND VRG AREEFFECTIVE PROCEDURES FOR THE SURGICAL TREATMENT OF CLINICALLY SEVERE OBESITY.NO MAJOR DIFFERENCES EXIST IN THEIR EFFECTIVENESS IN WEIGHT LOSS. THERE IS HOWEVER A DIFFERENCE BETWEEN THE TWO PROCEDURES, IN THE RATE OF POSTOPERATIVE COMPLICATIONS. VRG (ECKHOUT) IS ASSOCIATED WITH LESS STOMAL COMPLICATIONS, AND A BETTER FUNCTIONAL RESULT. WE BELIEVE THAT VRG IS THE PROCEDURE OF CHOICE BECAUSE IT IS TECHNICALLY SIMPLER, LESS TIME-CONSUMING AND MAINLY SAFER THAN THE VBG.
