    Urban-Rural and Socioeconomic Differences in Patient Knowledge and Perceptions of Genomic Tumor Testing

    Social determinants of health, such as rurality, income, and education, may widen health disparities by driving variation in patients\u27 knowledge and perceptions of medical interventions, and may be especially pronounced regarding medical technologies such as genomic tumor testing which may be less accessible and potentially misunderstood. Using date from surveys of patients with cancer enrolled in a large precision oncology initiative, the authors examined the knowledge and expectations and attitudes of these patients with toward large-panel genomic tumor testing, and analyzed differences by patient rurality, education, and income level. The authors found that patients’ education and income level are associated with knowledge, expectations, and attitudes toward genomic tumor testing, and rurality is associated with patient expectations. The authors conclude that efforts to promote adoption of genomic tumor testing therefore should focus on improving knowledge and awareness among individuals with low education and income

    Financial Alignment Initiative: New York Fully Integrated Duals Advantage for Individuals with Intellectual and Developmental Disabilities: Preliminary Third Evaluation Report

    The New York FIDA-IDD demonstration was launched in 2016 in nine downstate counties and was the first comprehensive managed care demonstration exclusively serving individuals with intellectual and developmental disabilities (IDD) in the nation. Due at least in part to a lack of provider participation, less than 8 percent of eligible beneficiaries enrolled. Beneficiaries who did enroll reported high levels of satisfaction, especially with care coordination and the ease of obtaining durable medical equipment. The MedicareMedicaid Plan’s (MMP) assessment and care coordination model provided person-centered care planning that identified goals and helped to achieve them, improving enrollees’ quality of life. The MMP successfully used telehealth and care coordination to reduce unnecessary trips to the emergency department. The low enrollment and enrollment of a frailer than expected population led to financial challenges for the MMP. The MMP was unable to spread start-up and administrative costs over a broad enrollment base, and expenditures for personal care and Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID) services outstripped the capitation rate paid to the plan. Overall, the demonstration had no impact on Medicare or Medicaid expenditures among all eligible beneficiaries over the first 4 demonstration years

    Description: Sage Hylton-Lemons grew up in Portland, Maine. In high school, Hylton-Lemons found sanctuary as a gay teenager in the youthdriven group Outright, where his involvement progressed from attending weekly meetings to being a member of the Board, then President of the Board. As an Outright member and activist, Hylton-Lemons spoke at schools and events, became a peer-advisor, and helped other communities organize their own Outrights. He was instrumental in conceiving and organizing Outright\u27s first annual prom in 1998. He was profoundly influenced by Christian T. Chenard, a nurse practitioner for the City of Portland Public Health Positive Healthcare Program. The papers include a scrapbook, photographs from the 1998 Outright Prom, a letter from the Maine Assistant Attorney General, a transcript of Hylton-Lemons\u27 testimony at a Portland City Council hearing regarding funding for Outright, and a transcript of his eulogy at the 2007 memorial service for Dr. Chenard along with other materials from the service.
