Fibrosi kistikoa duten pazienteen erizaintza-zainketak

Sarrera: fibrosi kistikoa (FK) gaixotasun genetiko autosomiko errezesiboa da, fibrosi kistikoaren mintz arteko eroankortasuna erregulatzen duen proteina kodetzen duen genean (CFTR) gertatzen diren aldaerek eragiten dutena, arnasketa- eta digestio- adierazpenak eraginez. Azken urteetan, paziente hauen bizi-kalitatean hobekuntza handiak lortu dira, bizi-itxaropena ere zeharo luzatuz. Orain arte, tratamendua sintomak arintzean oinarritzen zen eta gaur egun berriz, kaltetutako genean eragiten duten tratamenduak ikertzen ari dira. Hala ere, tratamendu hauek ikerketa prozesuan daude eta ez daude paziente guztientzat eskuragarri kostu handia dutelako. Beraz, sintomak arintzera bideratutako zainketak funtsezkoak dira FK dutenen bizi-kalitatea hobetzeko. Egungo ebidentzia tratamendu medikoan zentratuta dagoela ikusiz, erizainak ohiko tratamenduan zer funtzio dituen aztertuko da errebisio bibliografiko honetan. Helburua: Gradu Amaierako Lan honen helburua fibrosi kistikoa duten pazienteen erizaintza-zainketak identifikatzea da. Metodologia: errebisio bibliografiko bat egin da, Pubmed eta Dialnet datu-baseak eta Biblioteca Virtual en Salud metabilatzailea erabiliz. Iragazkiak erabiliz eta barneratze- eta kanporatze-irizpideak eta irakurketa kritikoa oinarri hartuz, artikuluen aukeraketa egin da, guztira 16 artikulu eskuratuz. Emaitzak: FKren sintomak arintzeko tratamendua funtsezko bost arlotan oinarritzen da: terapia antibiotikoa, nutrizioa, arnasketa-terapia, jarduera fisikoa eta laguntza psikosoziala. Antibioterapiari dagokionez, erizainak kateterra txertatu, antibiotikoak administratu eta pazientea zainketen inguruan hezi beharko du. Nutrizioaren alorrean, elikadura-egoera monitorizatu eta ohitura osasungarriak sustatuko ditu. Arnasketa- terapian, arnasbideak garbitzeko teknikak erakutsi eta arnas-funtzioa monitorizatuko du. Jarduera fisikoa sustatuz, pazienteak ariketa egokia egiten dutela bermatzen du, ariketa- planera egokituz eta motibazioa areagotuz. Azkenik, laguntza psikologiko eta soziala eskainiz, pazientearen eta familiaren sentimenduak kudeatzen laguntzen du eta tratamenduarekiko atxikimendua sustatzen du. Ondorioak: erizainak oinarrizko papera izango du FKren arlo guztien heziketan, pazienteak eta ingurukoek etxean burutu beharko dituzten zainketen eta ohitura osasungarrien jarraibideak emanez. Horrez gain, arrisku-faktoreak goiz detektatzeko gai izan beharko da eta pazientea ondo ulertzeko zaintza holistikoa bermatzen duen espazio bat sustatu beharko du, bai pazienteari eta baita ingurukoei ere arreta eskainiz. Characterization of visual cognition in pre-manifest, manifest and reduced penetrance Huntington’s disease

Cognitive and visual impairment are common in Huntington’s Disease (HD) and may precede motor diagnosis. We investigate the early presence of visual cognitive deficits in 181 participants, including HD carriers (40 pre-manifest, 30 early manifest, 27 manifest, and 6 reduced penetrance) and 78 healthy controls (HC). Significant differences in visual memory were observed between reduced penetrance and pre-manifest groups (p = .003), with pre-manifest showing worse performance. Age, education, CAG repeats, motor status, executive function, and verbal fluency, accounted for up to 72.8% of the variance in general and visual cognitive functions, with motor status having the strongest impact on visual domains in HD carriers. In pre-manifest HD, visual cognitive domains were primarily influenced by executive function, verbal fluency, age, and CAG repeats, while in early and manifest stages motor status and verbal fluency becomes more influential. ROC analyses showed that especially visuospatial abilities, visual memory, and visual attention (AUC = 0.927, 0.878, 0.874, respectively) effectively differentiated HC and pre-manifest from early and manifest HD. Early assessment of visual cognitive domains, particularly visual memory, could be an early marker of cognitive decline in HD. El análisis económico de las subastas ha tenido bastante éxito explicando aspectos fundamentales del comportamiento de los agentes que participan en ellas. Este libro presenta esas explicaciones y las utiliza para justificar los tipos y características de las subastas que se realizan en un conjunto amplio de problemas de asignación y, también, para señalar qué errores pueden haberse cometido en el diseño de algunas subastas. En el libro no solo se consideran las subastas (y licitaciones) de un bien o servicio: se estudian también, entre otras, las subastas de múltiples unidades de un mismo bien, las subastas simultáneas y las subastas consecutivas de unidades de un mismo bien o de varios bienes y servicios heterogéneos y las subastas combinatorias

“Discover your potential to raise your children”: A psychoeducational infancy parenting intervention in Spain

Background: The quality of parenting has a significant impact on healthy development across the life cycle. Recent research has highlighted the financial return of evidence-based parenting programs that are well designed and implemented. Objective: To analyze the impact of a psychoeducational guidance intervention program aimed at improving parenting skills. Method: Thirty-five families with children aged between 0 and 3.5 years referred from the educational, social, and health departments of the Vitoria-Gasteiz City Council (Spain) participated in the study. Using a longitudinal approach, the intervention took place over the course of three home visits: initial pretest assessment visit, feedback and orientation visit, and a final posttest assessment visit. Results: Statistically significant pretest–posttest differences were observed in parenting skills, including cognitive and socio-emotional development scaffolding, parental self-efficacy, parental adaptive stress, and diversity of experiences. Conclusions: Several areas for improvement were detected in the context of an intervention based on customized guidance for individual families. The implementation process indicated issues to be improved in future program editions. Implications: Our findings provide family and public health policy makers with empirical support for the implementation of parenting programs designed to help families to acquire and improve parenting competencies to foster their children’s comprehensive development, focused on the period between 0 and 3.5 years of life. Enhancement of thermal and mechanical properties of waterborne polyurethane-urea via chitin nanocrystal reinforcement

[EN] Chitin nanocrystals (ChNC) have attracted significant interest as reinforcement due to their exceptional me chanical properties such as Young's modulus, surface functional groups that promote hydrogen-bonding in teractions with specific matrices, and high length/diameter aspect ratio. Therefore, this study investigated the incorporation of chitin nanocrystals into a waterborne polyurethane urea dispersion (WBPUU) at concentrations ranging from 0.5 to 7 wt%. Nanocomposite films were prepared using an ultra-sonication assisted solvent casting method, and their properties were thoroughly analysed. The results demonstrate that the addition of ChNC significantly enhances thermomechanical stability, Young's modulus and stress at break, achieving the percolation threshold at a theoretical concentration of 3 wt% of ChNC. Notably, beyond the percolation threshold, these properties have increased significantly up to WBPUU7, with thermomechanical stability improving by more than 500 % and both Young's modulus and stress at break increasing by over 100 % when compared to values obtained at WBPUU. The study revealed that the addition of ChNC significantly influences water absorption, resulting in a fourfold increase in uptake in a basic medium. Furthermore, the abrasion esistance of the nanocomposites improves with ChNC content due to stable and interconnected network forms.Financial support from the Basque Country Government in the frame of Grupos Consolidados (IT-1690-22) is gratefully acknowledged. Microwave assisted Cobalt(III)-catalysed C-H aminocarbonylation reactions with isocyanates for the synthesis of thiophenecarboxamides

Sustituido el preprint por el accepted manuscript 02/09/2025A mild and efficient cobalt(III)-catalysed C-H aminocarbonylation procedure for thiophenes and benzo[b]thiophenes, using pyridine and pyrimidine directing groups to control site selectivity has been developed under MW assisted conditions. The reaction is effective with aromatic isocyanates with wide substitution patterns, although aliphatic isocyanates showed less reactivity. The introduced amide group can act as directing group in further iterative C-H functionalization reactions, allowing the diversification of the heterocyclic structures. Ampliación y desarrollo del gemelo digital de la UDEX mediante la integración de generación fotovoltaica y el sistema de almacenamiento en baterías

Ante la evolución del sistema eléctrico hacia redes más complejas, dinámicas y descentralizadas, los gemelos digitales se han convertido en una herramienta de simulación clave para anticiparse a distintos escenarios operativos y analizar la respuesta del sistema. Su uso permite reproducir condiciones reales de funcionamiento sin comprometer la seguridad de los equipos, facilitando el diseño, validación y optimización de nuevas soluciones tecnológicas antes de su implementación en campo. En este contexto, se ha ampliado y desarrollado el gemelo digital de la UDEX mediante la incorporación de una instalación fotovoltaica y un sistema de baterías al modelo preexistente. Para ello, se ha empleado el entorno Matlab-Simulink, recreando fielmente los principales componentes del sistema: módulos solares, inversores trifásicos, convertidores bidireccionales y baterías de ion-litio. Cada elemento ha sido modelado siguiendo fichas técnicas reales y condiciones representativas de operación. Esta ampliación mejora la capacidad de simulación del gemelo digital frente a escenarios con integración de renovables, incrementando así su valor como plataforma de validación previa a ensayos reales en la UDEX

Lau elkarrizketa erdi-egituratu burutu dira diziplina anitzeko ikuspegitik: Porrotx pailazoarekin, Txabi Arnal haur literaturako idazlearekin, Maider Angiozar hezitzailearekin eta Adur Berasategi dolu esperientzia goiztiarra izan duen gaztearekin. Aztertutako datuek erakusten dute haurrek dolua modu osasuntsuan prozesatzeko espazio seguruak eta hizkuntza egokia behar dituztela, eta irakasleek horretarako prestakuntza espezifikoa jaso beharko luketela.[ES] Gracias a su carácter metafórico y simbólico, la literatura infantil ofrece valiosas oportunidades didácticas y terapéuticas para ayudar a los niños a comprender la muerte, el duelo y los procesos de pérdida. Este Trabajo de Fin de Grado tiene como objetivo analizar la presencia de la muerte en la literatura infantil desde una perspectiva histórica y teórica, así como en el contexto actual de la literatura en euskera. Se han realizado cuatro entrevistas semi-estructuradas desde un enfoque multidisciplinar: con el payaso Porrotx, el escritor de literatura infantil Txabi Arnal, la educadora Maider Angiozar y el joven Adur Berasategi, quien ha vivido una experiencia de duelo temprano. Los datos analizados muestran que los niños necesitan espacios seguros y un lenguaje adecuado para procesar el duelo de manera saludable, y que el profesorado debería recibir formación específica para ello
